Job Description

A dynamic opportunity exists for a skilled Registered Nurse to join our team at Dartmouth Health, as a Patient Transition Specialist. This key role is designed to ensure seamless patient transitions across the care continuum, enhancing patient outcomes and satisfaction.

About the Role

This position requires a collaborative approach, working closely with multidisciplinary teams to develop safe transitional plans for patients transitioning from one level of care to another. Effective communication and advocacy are crucial in ensuring that patients receive optimal care, while also empowering them to manage their health effectively.

Key Responsibilities
  • Attend discharge planning meetings, reviewing medical records and gathering clinical information to inform transition planning.
  • Provide clear and concise information to patients and caregivers about post-acute discharge plans, promoting self-management skills and enhanced communications with healthcare providers.
  • Collaborate with the interdisciplinary team to develop comprehensive transition plans, ensuring that clinical and personal goals are met throughout the transition process.
  • Advocate on behalf of patients and healthcare providers for necessary resources and services, facilitating timely access to care.
Requirements

To be successful in this role, you will possess:

  • 3-5 years of experience in nursing, with a strong understanding of case management principles and practices.
  • A graduate degree from an accredited School of Nursing, along with a valid RN/LPN license in the State of NH.
  • Case Management certification from an accredited organization is highly desirable.
Benefits
