As a Physical Therapist with Penn Highlands Healthcare, you will be responsible for providing high-quality physical therapy services to patients in their place of residence. Key Responsibilities:
Provide physical therapy services to patients in their homes, promoting independence and improving overall health and well-being.
Collaborate with multidisciplinary teams to develop and implement effective treatment plans that meet the unique needs of each patient.
Conduct thorough assessments and evaluations to identify areas for improvement and develop strategies for achieving optimal outcomes.
Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
Requirements:
A degree from a baccalaureate, masters or doctorate program in physical therapy approved by an accredited organization.
Licensed to practice as a physical therapist in the State of Pennsylvania.
Appropriate experience as a physical therapist, with home health experience preferred.
Ability to work independently and within a multidisciplinary team, with excellent verbal and written communication and organization skills.
