Job Title: Certified Diabetes Educator

Bryan Health is seeking a highly skilled Certified Diabetes Educator to join our team. As a key member of our healthcare team, you will play a vital role in providing expert diabetes education and self-management skills to patients of all ages.

Key Responsibilities:
  • Assess patient needs and develop personalized education plans to address their unique barriers to learning and goals.
  • Provide comprehensive diabetes education and training to patients and their families, incorporating cultural considerations and evidence-based interventions.
  • Collaborate with healthcare teams to define and achieve patient and family goals, and identify and resolve barriers to care.
  • Develop and maintain strong relationships with physicians and hospital staff to ensure seamless care coordination.
  • Participate in data collection and analysis to inform program evaluation and improvement.
  • Facilitate and coach clinical staff in the implementation of patient education and discharge planning.
Requirements:
  • Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence.
  • Minimum of two years experience in clinical practice required.
  • Certified Diabetes Educator certification required at hire or within one year of hire date.
