We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Home Care Nurse Specialist in Dayton, OH.

Job Description:

The Home Care Nurse Specialist will play a critical role in providing high-quality nursing care to patients in their homes, working collaboratively with an interdisciplinary team to achieve positive patient outcomes. Key responsibilities include:

  • Providing skilled nursing care as ordered by the attending physician and monitoring patient conditions to notify case managers of any changes.
  • Administering medication as prescribed and educating patients on preventative measures to minimize risk and re-hospitalizations.
  • Educating patients and families on disease processes using teach-back methods to ensure understanding.
Requirements:

To be successful in this role, you will need:

  • A current LPN license in the state of employment.
  • Minimum one year of experience as a LPN in an acute care setting, with home care experience preferred.
  • A valid driver's license and auto insurance.
Compensation:

The estimated salary for this position is $62,000 - $82,000 per year, depending on experience and qualifications. We offer a competitive benefits package, including medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
