About the Role
As a RN Care Coordinator at CommonSpirit Health, you will play a vital part in ensuring seamless transitions of care for patients. With a strong focus on quality and cost-effective outcomes, this role requires collaboration with various stakeholders to achieve optimal results. The ideal candidate will possess excellent communication skills, be able to analyze complex situations, and prioritize tasks effectively.

Key Responsibilities
The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for identified patients. This involves promoting quality care, following hospital policies and Federal regulations, and collaborating with utilization management, nursing, physicians, and ancillary departments. Additionally, the RN Care Coordinator advocates for patient choice, spiritual needs, cultural language, and socioeconomic barriers to care transitions. A crucial aspect of this role is enhancing the patient experience.

Requirements
To succeed in this position, you must have a minimum of two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing. California RN licensure, AHA BLS card, and ability to pass annual Inter-rater reliability test are also essential. You should be able to apply clinical guidelines, possess critical thinking and problem-solving skills, collaborate effectively with multiple stakeholders, and have professional communication skills.

Preferred Qualifications
A graduate of an accredited school of nursing (Bachelor's Degree in Nursing) or related healthcare field is preferred. At least five years of nursing experience, Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification, and knowledge of managed care and payer environment are highly valued.
