Job Description

We are seeking a compassionate and skilled Home Health Nurse to provide high-acuity care for a pre-teen patient in their home. This is a unique opportunity to make a meaningful difference in the life of a special individual.

Responsibilities:

  • Implement the prescribed plan of care from the physician
  • Patient assessments and coordination of care with the medical team
  • Administration of prescribed medication, treatments, and therapies as ordered
  • Compliant documentation of care provided in our system of record
  • Health promotion, teaching, and training of family members on patient care
  • Acceptance and writing of verbal orders from physicians and obtaining co-signature of Case Manager
  • Frequent communication with the physician, client, family members, staff, and Case Manager on client problems, concerns, and changes in client condition

Qualifications:

  • Active, unencumbered license (LPN) in the state where you will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test within the last 12 months
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation for traveling to patient locations
