Marin City Health and Wellness Center (MCHWC) is a Federally Qualified Health Center (FQHC) that provides innovative health and wellness services to all, with the goal of African American health equity.
The LVN-II Addiction and Substance Use Services is a senior LVN position primarily attached to our Addiction and Homeless Services Team, working in close coordination with our Behavioral Health and Medical providers, for the care of patients who either: 1) have identified substance use disorders and are/will be receiving MAT services; 2) are at high risk for substance use issues without an existing diagnosis; 3) are without concern for substance use issues but are on high-risk psychiatric or controlled medications.
Marin City Health and Wellness (MCHWC) is an eligible Federally Qualified Health Center in the National Health Service Corps Loan Repayment Program.
Essential Duties and Responsibilities:
Duties may include but are not limited to the following:
- Assess clients' current condition, health problems, and/or needs, including seizure risk, level of cognition, vital signs, and suicidal/homicidal ideations.
- Assess, monitor, and provide necessary interventions for each client, considering drug intoxication, withdrawal, and preexisting conditions.
- Anticipate clients' needs based on their drug of choice, previous withdrawal history, pre-existing conditions, and current status.
- Monitor and provide interventions for clients in the event of medication side effects, including adverse drug reactions.
- Assess and monitor barriers to learning while providing education, keeping these barriers in mind.
- Demonstrate the ability to establish a professional rapport with clients, families, and other caregivers.
- Assess and monitor clients through standardized scales like COWS, CIWA, SAWS.
- Safely and properly draw up and administer therapeutic injections according to best practices and clinic protocols.
- Safely and correctly collect, handle, and result point-of-care testing, like urine tests and oral swabs.
- Document accurately and thoroughly in notes in accordance with established policies and procedures, including in a timely manner and legibly.
- Demonstrate knowledge of detoxification protocol and medications used in the detoxification process, including mechanism of action, side effects, and drug interactions.
- Provide safe and effective care for clients identified to be at risk of injury to self or others and report findings and client's response accurately to the clinician and the client's treatment team.
- Interact with clients in accordance with the goals and interventions outlined in their respective treatment plans and specific needs.
- Demonstrate the ability to interact appropriately with clients and coworkers at all times.
- Demonstrate the ability to therapeutically respond to clients and their family members as indicated.
- Communicate effectively with outside clinicians and other appropriate parties in order to plan, deliver, and evaluate the client's response to care.
- Ensure client compliance with facility rules and guidelines and report adequately.
- Report incidents of suspected/known abuse/neglect to the Administrative Director and Program Director, complying with all mandatory reporting laws.
- Maintain HIPAA standards/laws in all aspects of their duties.
Qualifications:
- Must be licensed (LVN, MAT, and/or RN) in the State of California, in good standing.
- Minimum 2-5+ years' experience in a mental health facility or;
- Minimum 2-5+ years' experience with Medically Assisted Treatment (MAT) preferred.
- Familiarity with mental health community services and resources.
- Ability to work independently with minimal supervision.
- Should possess a professional demeanor and a collaborative spirit.
- Excellent oral and written communication skills.
- Ability to recognize problems, collect data, establish facts, and follow up with patients.
- Ability to understand and apply guidelines, policies, and procedures.
