Job Description

As a Resource Navigator at Community Passageways, you will partner with the Community Safety Team to form a Care Team that provides intensive wraparound services to participants. Your role will involve relationship building, coaching/mentoring, safety planning, strength and need assessments, goal planning, coordination of community services, and systems navigation and advocacy.

Key Responsibilities
  • Intake, Assessment, and Goal Setting
    • In coordination with the Community Safety Team, provide one-on-one outreach to youth and young adults between the ages of 12 to 30 who meet three risk factors and/or those referred through SCSI and RPKC network.
    • Connect with participants and establish a supportive relationship.
    • Review service plans with participants every 60 days and edit service plans as needed.
  • Program Support
    • Connect participants to comprehensive wrap-around services such as housing, employment, medical support, mental health, substance use, legal advocacy, etc.
    • Schedule appointments for participants and work with Community Safety Team Members to coordinate transportation as needed.
    • Develop and maintain relationships with systems players with whom participants interact.
