Job Title:

Physical Therapist - Home Health Specialist

Job Summary:

We are seeking a skilled Physical Therapist to join our team as a Home Health Specialist in Alameda County. As a Physical Therapist, you will be responsible for providing high-quality patient care in a home health setting.

Responsibilities:
  • Prioritize start of care visits to ensure timely patient care
  • Supervise Physical Therapy Assistants to ensure efficient patient care
  • Report start of care visits to the supervisor on the next business day
  • Document clinical notes and other relevant information within 48 hours of each visit
  • Maintain open communication with the supervisor, intake, and office staff
  • Be responsive during normal business hours
Requirements:
  • Current CA PT License
  • At least one year of experience in Home Health
Work Environment:

This position is based in Alameda County and requires a flexible schedule, including one weekend a month rotation. The territory will be required to expand as needed.
