About the Role
We're currently seeking experienced RN Case Managers to join our team in the Greater Seacoast Area. As a Home Health RN, you'll be responsible for managing a defined caseload of patients, providing direct patient care, and delivering comprehensive nursing services to individuals in the community. Your expertise and compassion will make a tangible difference in the lives of our patients and their families.
Key Responsibilities
- Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals to coordinate patient care and services.
- Conduct regular patient assessments to monitor progress and adjust care plans as needed.

Requirements
To be considered for this opportunity, you'll need to possess:
- A valid nursing license in the State of New Hampshire.
- At least two years of experience as an RN in a healthcare setting.
- Current CPR certification.
- Reliable transportation to travel to patient homes and healthcare facilities.
