Inpatient Nutritionist Clinical Nutrition Mount Sinai Hospital Full Time

We are seeking a highly skilled Inpatient Nutritionist to join our Clinical Nutrition team at Mount Sinai Hospital. As a key member of our team, you will provide quality nutrition care and education to patients and their families, working under the direction of our Clinical Nutrition leadership.
Responsibilities- Evaluate the nutritional status of patients by reviewing pertinent information from the electronic medical record and the interdisciplinary healthcare team.
- Apply knowledge of nutrition screening and assessment to assigned patient populations to determine their nutritional needs.
- Conduct nutrition-focused physical exams to determine risk and/or degree of malnutrition.
- Document pertinent findings in the medical record as per policy.
- Maintain individual daily productivity, annual competencies, and other required documentation of direct and non-direct patient care functions, with timely submission to direct supervisor.
- Meet department benchmarks for productivity.
- Provide quality nutrition counseling to patients, family members, and caregivers as indicated.
- Evaluate nutrition care plans for effectiveness and recommend changes as needed.
- Use professional judgment to adapt and apply guidelines, protocols, and professional standards of care to the new and changing needs of the patient.
- Evaluate the need for enteral and parenteral nutrition support.
- Obtain patients' food preferences and confirm allergies. Communicate with the diet office as needed. Perform meal rounds.
- Work collaboratively and professionally with the Food Service Department to ensure patient needs and compliance to therapeutic diets are met.
- Communicate effectively with members of the healthcare team by participating in interdisciplinary rounds and discharge planning rounds.
- Precept dietetic interns and instruct them on the competencies required to complete their assigned rotation. Provide feedback for incorporation into student evaluations.
- Participate in department activities, including nutrition-related in-services, interdepartmental nutrition programs, lectures, and community-focused programs.
- Demonstrate effective communication with peers and supervisors.
- Accept and incorporate feedback for clinical development.
- Compliant with hospital/regulatory guidelines and policies.
- Work rotating weekends and holidays.
- Perform other duties as assigned.
