Key Responsibilities:
  • Maintain confidentiality and comply with company, state, federal and HIPAA rules and regulations
  • Charting for insurance purposes
  • Maintain a positive, professional attitude toward patients, staff, and volunteers
  • Create a safe and healthy environment for clients
  • Have a sincere commitment to patient's recovery
  • Transport clients to appointments when necessary
  • Monitor client's emotional well-being and behavior
  • Connect client with family and community resources
  • Connect client with legal entities when necessary
  • Motivate clients toward goal achievement
  • Meet with clients weekly
  • Help residents with long-term care plan development and educational resources
About You

We are looking for an individual with a Bachelor's degree in social work or human services field and 1 year post-grad work experience or Master's Degree required. Additionally, you must have a current driver's license, basic computer skills, ability to complete and submit reports, be self-motivated, and possess excellent documentation and communication skills.
