PRN Physical Therapist for Topeka Assisted Livings

We are seeking an experienced Physical Therapist who can evaluate and treat patients in accordance with orders from the physician through the use of therapeutic procedures designed to address deficits in physical function and improve independence, safety, and quality of life.
The ideal candidate will be responsible for ensuring quality, efficient physical therapy is provided to patients by managing the delivery of care throughout the course of treatment and delegating appropriate components of care to the Physical Therapist's Assistant under their supervision.
In this role, you will participate as a member of the interdisciplinary team providing support and information within the area of physical therapy. Your primary responsibilities will include:
- Evaluating available patient information and assessing its impact on the assessment and treatment process.
- Conducting and documenting thorough functional assessments of patients using standardized procedures, including home/vocational and equipment needs.
- Developing and implementing individualized treatment plans based on patient needs and containing achievable functional goals.
- Complying with evaluation, treatment, and documentation guidelines.
- Recording patient progress in the patient record a minimum of every seven days.
- Revising treatment plans, as needed, throughout the treatment process.
- Documenting the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
- Orienting patients to program services and treatment procedures appropriate to their needs and informing them of any potential risks during procedures.
- Working with nursing staff, patients, and families to maximize patients' use of functional abilities.
- Establishing and instructing patients and family members/caregivers in exercises and therapeutic procedures to be continued following discharge.
- Assisting in the referral process when patients require additional services following discharge and maintaining current knowledge of community resources.
- Attending and participating in rehabilitation team conferences regarding patient progress, problems, or needs.
- Attending and participating in other department/facility meetings, as required.
- Accessing continuing education opportunities appropriate to discipline and responsibilities to maintain clinical competency.
- Participating in the quality improvement process by responding appropriately to results of medical record audits, patient/referral source satisfaction surveys, safety activities, and regulatory requirements.
- Acting in compliance with Vertis Therapy Management policies and procedures, State, Federal, and Professional Organization regulatory and professional standards and guidelines.
- Providing input to the Rehab Director on clinical performance of assistants as requested.
- Promoting the programs and services of Vertis Therapy through formal and informal interactions with the community.
The estimated salary for this position is $90,000 - $120,000 per year, depending on experience. We offer a competitive compensation package, including benefits and opportunities for career growth.
