Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.

Responsibilities
  • Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes
  • Work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
  • Collaborate with primary care physicians on patient education and provide follow-up care as needed
Requirements
  • Master's degree in nursing or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Current BLS, ACLS, or CPR certification
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health/medical record software
  • Excellent verbal and written communication skills with patients, clients, and colleagues
  • Ability to travel and drive a car, with proof of adequate automobile insurance coverage
