Job Description:
We are seeking a skilled Nurse Practitioner to join our team as a Skin and Wound Team Lead. This role involves leading and developing collaborative multidisciplinary wound care teams, traveling to scheduled locations for wound rounds and house calls, and collaborating with medical leadership to achieve the best possible outcomes.
Your Key Responsibilities:
- Lead and develop collaborative multidisciplinary wound care teams
- Travel to scheduled locations weekly for wound rounds and house calls in an assigned region
- Provide high-quality skin and wound care services to patients
- Order, interpret, and evaluate laboratory and diagnostic studies to assess changes to patients' healthcare needs
- Perform bedside wound care skills, including wound assessment, dressing application, pressure relief, pain/odor control, and minor surgical procedures
- Track the progress of wound healing and document timely in UWH's own wound care EMR
- Develop treatment plans for skin and wound conditions based on individual patient needs and UWH Clinical Practice Guidelines
- Collaborate virtually with medical leadership, including board-certified physicians and surgeons specializing in wound care, dermatology, podiatry, vascular surgery, and infectious disease
