Job Summary

We are seeking a highly motivated Therapist to join our team. As a key member of our interdisciplinary team, you will be responsible for assessing patients' strengths and weaknesses to create effective treatment plans aimed at healing, recovery, and maximizing functional independence.

Your Key Responsibilities:

• Conduct thorough patient evaluations to identify areas of improvement
• Develop and implement personalized treatment plans to achieve optimal patient outcomes
• Collaborate with an interdisciplinary team to ensure seamless care coordination
• Provide education and training to patients and their support systems to promote healthy habits and independence
• Maintain accurate records and reports to meet regulatory requirements
• Stay up-to-date with industry developments and best practices through ongoing education and professional development

• We offer a competitive salary range of $65,000 - $90,000 per year, depending on experience

Requirements:

• Bachelor's or Master's degree in Physical Therapy (PT)
• Licensed PT in the state of practice
• Valid CPR certification
• Experience working with EMR software preferred
• Excellent communication and interpersonal skills
• Strong clinical decision-making skills
