Rehabilitation Specialist

Responsibilities:
* Create a safe and trusting space to build rapport with participants and their designated supports
* Complete various assessments, treatment plans, reviews, and progress notes in a timely manner
* Effectively collaborate and communicate with treatment providers and staff
* Maintain confidentiality of verbal and written communication
* Adhere to HSI policies and procedures as well as the NASW Code of Ethics
* Effectively navigate Electronic Medical Record systems (Athena. Training will be provided in house)
* Complete 10 CEUs annually

Pay:
* $20.00 to $24.50 per Billable Hour.
* Compensation is determined based on licensure, and experience.
