Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to conduct home risk assessments and make a direct impact on the health and well-being of others.

This PRN position offers flexible scheduling and competitive compensation.

You will work collaboratively with physicians, case managers, social workers, and family members to provide comprehensive care.

Key Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
Requirements

To be successful in this role, you must have:

  • A Master's Degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • A current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
Travel Requirements

As a travel-friendly organization, we require our providers to have:

  • A valid state driver's license and ability to drive a car.
  • Proof of adequate automobile insurance coverage for the state of residence.
Salary and Benefits

Enjoy a competitive salary of $95.00 or $110.00 per completed assessment, depending on county, with flexible scheduling options available.

Mileage reimbursement is provided for all miles when 20 or more completed assessments are completed monthly.

No show payment is available for 10 or more booked assessments per month.
