Advanced Social Worker Lead for Hospital-Based Case Management

We are seeking a highly skilled Advanced Social Worker Lead to join our team at Houston Methodist Cypress Hospital. As a key member of our case management team, you will play a critical role in ensuring seamless transitions for patients and their families, leveraging your expertise in social determinants, community resources, and psychosocial assessments.
Key Responsibilities- Lead comprehensive case management activities, including high-risk patient identification, daily care coordination rounds, and resolution of barriers to efficient patient throughput.
- Conduct thorough assessments, addressing social determinants, and develop effective discharge plans, coordinating with the interprofessional healthcare team, patients, and families.
- Collaborate with staff from various disciplines to improve outcomes and ensure safe transitions through effective patient handoffs.
- Identify opportunities for professional growth and development among colleagues, providing education and support as needed.
- Develop and maintain relationships with community resources, regulatory agencies, and payers to optimize patient care and outcomes.
- Promote evidence-based practice, participating in unit projects, and contributing to the advancement of social work standards and competencies within the organization.
- Masters degree in Social Work (MSW) from an accredited university.
- Three years of experience in social services within a healthcare setting.
- Licensure as a Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW), along with specialized certifications such as ACM-SW, CCM, or C-SWHC.
- Demonstrated skills in communication, collaboration, negotiation, mediation, time management, prioritization, and adherence to clinical practice standards.
- Proficiency in Microsoft Office Suite, particularly Access, Excel, Outlook, PowerPoint, and Word.

Salary and BenefitsWe offer a competitive salary range of $85,000 - $110,000 annually, depending on qualifications and experience.
