Job Summary

We are looking for a skilled Occupational Therapist to provide home health services to our patients. As an Occupational Therapist with Elara Caring, you will have the opportunity to make a real difference in the lives of our patients and their families.

Responsibilities
  • Provide occupational therapy services to patients in their homes, including assessments, goal development, and treatment plans.
  • Collaborate with other healthcare professionals to ensure comprehensive care for our patients.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Conduct home safety assessments and provide education to patients and their families on safe home practices.
  • Document patient progress and communicate with other healthcare professionals as needed.
Requirements
  • Current OT License as required by state.
  • CPR certification with American Heart Association or American Red Cross.
  • 1 year of experience as an Occupational Therapist.
  • Ability to meet physical demands of the job.
  • Positive attitude and dedication to quality patient care.
  • Strong communication skills and ability to work effectively with patients, families, and other healthcare professionals.
