Job Summary

This position is responsible for providing professional/clinical services to patients in inpatient Psychiatry to assess and address the need for psychiatric and/or substance abuse treatment, psychiatric medication(s), screening and referral for medical care, and follow-up as appropriate.

Key Responsibilities

1. Performs a comprehensive psychiatric evaluation that includes assessing current psychiatric symptoms and past psychiatric history, the psychosocial and physical health status, evaluation of mental status, current and past history of violence, suicidal or self-harm behavior, substance use, level of functioning, health behaviors, trauma, and social and developmental history.
2. Completes Initial Psychiatric Assessment, Suicide Risk Assessment, AIMS, and appropriate rating scales on the first treatment day with patient.
3. Reviews the medical record if the patient has had prior hospitalizations,
4. Contacts the patient's outpatient psychiatrist (or clinician) to exchange clinical information if patient has been treated outside of hospital.
5. Documents patient's clinical and treatment status daily for first five working days and a minimum of three times a week thereafter. There must also be a note when there is a significant change in the patient's condition or treatment.
6. Documents any psychiatric, behavioral, substance abuse and active medical problems on the Problem List and Comprehensive Treatment Plan within three days of admission.
7. Develops a differential diagnosis derived from the collection and synthesis of assessment data.
8. Consults with other members of the interdisciplinary team to determine, evaluate, and plan treatment goals/objectives,
9. Updates and completes the Treatment Plan Review weekly after the completion of the Comprehensive Treatment Plan and/or whenever there is a significant change in the patient's condition or treatment.
10. Records patient's status and progress of care rendered, and plan for future care and therapeutic interventions appropriate to the patient's needs.
11. Counsels and guides patients, their families, and/or significant others relating to physical health, psychiatric treatment, and progress,
12. Request appropriate laboratory tests within established protocols, and interprets findings of relevant diagnostic and laboratory tests.
13. Prescribes and makes adjustments on medication within established protocols, and follow such medication(s) interventions as appropriate'
14. Manages psychiatric emergencies by determining the level of risk and initiating and coordinating effective emergency care.
15. Serves as team leader in psychiatric crisis situations.
16. Ensures that patient and staff debriefings are conducted immediately following Code Greys or other behavioral crises, leading the debriefings if possible'
17. Ensures that a patient Behavioral/Safety Plan is in place for all patients, and that team members are aware of and consider patient preferences in responding to a crisis.
18. Consults with the physician, as appropriate, for evaluation of findings of the psychiatric and health screening in order to make prospective decisions about treatment.
19. Ensures the timely and appropriate management of a patient's medical problems by the unit's internist.
20. Notifies the Unit Chief and/or Director of Inpatient Psychiatry of sentinel events, incidents, Code Greys, elopements, deviations from standards of care and problems that may affect the customary and usual activities of the unit'
