Job Summary:

The Discharge Liaison Coordinator plays a vital role in ensuring seamless transitions for patients leaving our hospital. Under the direction of RN Case Managers or Social Work staff, this individual will arrange post-acute services, communicate with patients and families, and complete tasks assigned by the case management team.

Key Responsibilities:

  • Arrange post-discharge services, taking into account patient preferences and Tenet policies
  • Create and follow up on electronic referrals using the Tenet Case Management system
  • Review patient choice letters with patients/families, obtaining required signatures
  • Provide important information to Medicare patients prior to discharge
  • Communicate effectively with patients, families, and healthcare teams
  • Complete tasks assigned by Case Manager and Social Work staff, including making copies, sending faxes, and phone calls

Qualifications:

  • High school diploma or equivalent
  • 2 years of experience in a clerical or healthcare field (acute hospital experience preferred)
  • Preferred: Paramedic, EMT, or Certified Nursing Assistant certification

Tenet complies with federal, state, and local laws regarding mandatory vaccination of its workforce.
