Job Summary

We are seeking a reliable and detail-oriented MDS Coordinator to join our team. As a MDS Coordinator, you will be responsible for completing all required MDS assessments timely and accurately, providing oversight and education to the interdisciplinary team, and tracking Medicare benefit days.

Key Responsibilities
  • Complete all required MDS assessments (admission, 5 day, annual, change in condition, and discharge MDS) timely and accurately.
  • Provide oversight and education to the nursing and interdisciplinary team regarding MDS protocols and accurate care planning interventions.
  • Track Medicare benefit days and validate daily Medicare census.
  • Coordinate information with the business office.
  • Case management for non-Medicare short stay residents (HMO, Private Insurance).
Requirements
  • CA Licensed RN or LVN with current license in good standing.
  • Minimum 2 years experience as a MDS Coordinator in a Medicare Skilled Nursing Facility including MDS 3.0 experience.
  • Strong knowledge and working experience with State and Federal regulations, the RAI (Resident Assessment Instrument), and PDPM criteria.
  • Computer proficient with Microsoft Word, Outlook, and EMR system (preferably Point Click Care).
  • Legally Authorized to work in the United States.
