We are seeking an experienced Registered Nurse (RN) to join our team as a Patient Care Coordinator Senior. In this vital role, you will play a key part in delivering high-quality patient care and fostering collaborative relationships with healthcare providers and staff members.

Job Summary
In this position, you will be responsible for providing daily nursing care and education to patients, as well as collaborating with the Center Manager to identify opportunities for improving clinical performance and implementing effective solutions. Your expertise will help ensure compliance with national and organizational standards of care, maintaining current knowledge of state regulations and best practices in healthcare.

Responsibilities
* Provide direct patient care, including phone and in-person triage, and patient education
* Collaborate with Providers and Center Manager to improve clinical performance and implement performance improvement activities
* Ensure compliance with national and organizational standards of care
* Maintain current knowledge of state regulations and best practices in healthcare
* Identify and provide resources for patient education and training

Requirements
* AA/AS degree in Nursing required; BA/BS degree in Nursing or Healthcare Administration preferred
* Valid, active Registered Nurse (RN) license in state(s) of employment required
* A minimum of 5 years', current direct nursing/medical work experience required
* Experience working in community-based clinics, public health departments, or specializing with the senior population is preferred
