About the Role

We are seeking a highly skilled and compassionate Family Preservation Services Case Manager to join our team in Albemarle, Virginia.

Key Responsibilities
  • Partner with families through In-Home Services cases to develop and implement family-specific psychosocial case plans that increase child safety, reduce the risk of abuse/neglect, and foster care, while building on family strengths.
  • Support families in accessing resources and services that can reduce risk and build protective capacity through providing services and conducting assessments from a strengths-based, trauma-informed, culturally humble, and family-centered perspective.
  • Offer clinical case management to children and families that applies family-systems treatment models to strengthen families and prevent foster care through ongoing assessment, planning, intervention, collaboration, and evaluation.
  • Utilize best practices in family engagement and family finding to identify and connect children and families with their natural supports, increasing child safety, well-being, and permanency.
  • Participate in multi-disciplinary and interagency teams providing services to customers, collaborating with community partners such as schools, courts, and local therapeutic providers, and coordinating services through home visits, school visits, and team meetings.
Requirements
  • Minimum of a bachelor's degree in a Human Services field or a minimum of two years of experience in a Human Services area.
  • Working knowledge of social work principles and practices, current social, economic, and health problems, social institutions and community resources, and the methods of the helping process.
  • Skill in critical thinking, crisis intervention and de-escalation, decision-making and independent judgment, and using personal computer software.
  • Ability to identify social problems and client strengths and needs, develop rapport with clients from diverse backgrounds, recognize and accept the rights, responsibilities, and differences of others, and work effectively with individuals and families to help them attain success.
Salary and Benefits

The estimated salary for this role is between $65,446.45 and $72,335.55 per year, depending on qualifications and experience.
