Nurse Practitioner

Trinity Health, a not-for-profit Catholic health system, seeks an experienced Nurse Practitioner to join our team in providing exceptional patient care. As a key member of our healthcare team, you will work collaboratively with physicians and other healthcare professionals to deliver high-quality, patient-centered care.
About the Role:
We are seeking a compassionate and skilled Nurse Practitioner who is committed to delivering excellent patient outcomes. In this role, you will assess patient needs, develop individualized care plans, and implement evidence-based treatments to promote optimal health and well-being. You will also collaborate with our interdisciplinary team to ensure seamless transitions of care and coordinate services as needed.
Key Responsibilities:
- Assessment and Diagnosis: Conduct thorough assessments of patients to identify their unique needs and develop comprehensive care plans that address their physical, emotional, and social requirements.
- Treatment Planning and Implementation: Develop and implement individualized treatment plans based on evidence-based best practices, taking into account patient preferences and values.
- Care Coordination: Collaborate with our interdisciplinary team to ensure seamless transitions of care, including referrals to specialists, home health services, and community resources.
- Patient Education and Support: Educate patients and their families about disease management, medication adherence, and healthy lifestyle choices to empower them to take an active role in their care.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes, improve clinical practices, and reduce readmissions.
Requirements:
To be successful in this role, you must possess a strong foundation in nursing practice, excellent communication and interpersonal skills, and a commitment to delivering exceptional patient care. We require:
- A Bachelor of Science in Nursing (BSN) degree or higher from an accredited institution.
- An active RN license in the State of Iowa.
- Board certification in a relevant specialty (e.g., family medicine, pediatrics, gerontology).
- Experience working as a Nurse Practitioner in an acute care setting.
- Excellent communication, teamwork, and problem-solving skills.
Salary and Benefits:
We offer a competitive salary range of $120,000 to $150,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision insurance, retirement savings plan, paid time off, and continuing education opportunities.
