Hospice Nurse Specialist

The salary for this position is estimated to be around $65,000 annually, based on industry standards and location.
We are committed to hiring qualified professionals who share our passion for hospice care. As a Licensed Practical Nurse (LPN), you will play a vital role in our interdisciplinary team, working closely with patients, families, and healthcare providers to ensure seamless care.
Job Responsibilities:
- Provide direct patient care as defined by the State Nurse Practice Act
- Implement current nursing practices following comprehensive assessments and plans of care
- Document patient services accurately and timely to reflect the plan of care
- Educate patients and families/caregivers on diagnosis and self-care plans
- Participate in coordinating hospice services and identifying patient needs
- Manage pain and symptoms effectively, evaluating patient responses to treatments/medications
Requirements:
- Graduate of an accredited practical nurse or vocational nursing program
- Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings
- Current LPN/LVN licensure in the practicing state
- CPR certification
- Demonstrated excellent verbal and written communication, organization skills
- A valid driver's license with insured automobile
- Understanding of the hospice concept and terminally ill patients' needs
