About the Role:

Accompany Health is seeking a highly skilled Behavioral Health Advanced Practice Clinician to join our team. As a key member of our Integrated Care Team, you will work closely with our Director of Integrated Behavioral Health to provide high-quality, patient-centered care to individuals with complex behavioral health needs.

As a Behavioral Health Advanced Practice Clinician, you will be responsible for providing psychiatric diagnostic evaluations, prescribing medications, and designing patient treatment plans. You will also collaborate with our multidisciplinary care team to coordinate patient care and ensure seamless transitions.

Responsibilities:
  • Provide psychiatric diagnostic evaluations for patients referred to BH services.
  • Prescribe medications for depression, anxiety, PTSD, bipolar disorder, schizophrenia, substance use disorders, and other DSM5 diagnoses.
  • Design and participate in patient treatment plans, particularly for complex patients.
  • Collaborate with Psychiatrists, Behavioral Health Clinicians, Primary Care APCs and Physicians, Care Team Managers, Community Health Workers, Patient Experience Navigators, and other team members to coordinate patient care.
  • Support patients in transitions of care from facilities for admissions related to behavioral health diagnoses.
Requirements:
  • Certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with Psychiatry CAQ.
  • Active DEA Controlled Substance Registration or eligible for application.
  • Active Unrestricted license to practice as an advanced practice registered nurse (APRN) or Physician Assistant in the state of Michigan.
  • Active controlled substance license in Michigan.
  • Certified in Basic Life Support for Healthcare Providers.
