Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at City of Hope, providing exceptional care to geriatric and adult patients.

This is a unique opportunity for an experienced Nurse Practitioner to work in a fast-paced environment, delivering high-quality patient care and leading by example. The successful candidate will be responsible for:

• Providing individualized care to geriatric and adult patients, utilizing the nursing process to assess, diagnose, plan, implement, and evaluate patient outcomes;
• Developing and implementing individualized plans of care, taking into account the physical, spiritual, emotional, and educational needs of patients;
• Collaborating with other healthcare professionals to coordinate patient care and ensure effective communication throughout the care continuum;
• Maintaining accurate and up-to-date records, adhering to regulatory requirements and enterprise policies/procedures;
• Participating in quality improvement initiatives to enhance patient outcomes and satisfaction.

To be considered for this role, you should have:

• A Master's degree in Nursing and current licensure as a Nurse Practitioner in the state of IL;
• At least 1-2 years of experience in acute care settings, with a focus on geriatric care;
• BLS certification and ACLS certification (as required by population served);
• Ongoing education and professional development opportunities, including training in advanced life support and leadership skills;
• Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
