We are seeking a highly skilled and dedicated Medical Director of Skilled Nursing to join our team at Afton Oaks Nursing & Rehab Center. As a key member of our interdisciplinary team, you will play a critical role in fostering an environment of Service Excellence and delivering exceptional patient care.
The ideal candidate will possess a strong background in long-term care, with experience in facilitating the RAI Process and coordinating care plans. You will work closely with our interdisciplinary team to identify patient needs, ensure timely MDS assessments, and maintain accurate coding of MDS assessments.
Responsibilities will include:
- Facilitating the RAI Process under the direction of an R.N. or DNS
- Collaborating with the interdisciplinary team to identify patient needs
- Coordinating and conducting daily Case Management meetings
- Ensuring timely MDS assessments according to state and federal regulations
- Ensuring accurate coding of MDS assessments
- Maintaining the tracking system of MDS assessment schedules
- Facilitating Care Plan conferences with the interdisciplinary team, patient, residents, and families
- Obtaining, reviewing, and maintaining all State and Federal reports
- Monitoring Quality Measures and ensuring MDSs are accurate to support and reflect Quality Measures
- Providing education related to the RAI Process
- Coordinating and completing electronic submission of required documentation
- Ensuring Medicare and Medicaid regulatory guidelines are completed accurately and timely
- Providing Managed Care case management at the center level
- Reviewing Additional Documentation Requests with the Health Information Management Coordinator
- Participating in billing reviews to ensure claims have supporting MDS assessments documents
- Participating in Quality Assurance and Process Improvement Activities
- Continuing to update knowledge base related to Medicare, Managed Care, Medicaid, RAI Process, and computer technology updates
