Mental Health Case Manager

We are seeking a skilled Mental Health Case Manager to join our team at Service Access & Management, Inc. The successful candidate will play a critical role in coordinating and advocating for the mental health needs of our clients.
Key Responsibilities:
- Conduct initial assessments to determine client needs and develop comprehensive treatment plans.
- Coordinate with clients, families, and team members to ensure effective service delivery.
- Connect clients with appropriate mental health services and resources.
- Monitor client progress and provide onboarding support and guidance.
- Respond to mental health crises and implement interventions to ensure client safety.
- Advocate for clients' rights and ensure access to necessary resources.
Requirements:
- Bachelor's degree in psychology, social work, counseling, or related field.
- 2 years of experience in mental health or a related field.
- Valid Pennsylvania driver's license and automobile insurance.
Working Conditions:
Prolonged periods working on a computer in an open office environment. Ability to travel throughout the region and lift up to 15 pounds.
