We are seeking a skilled Licensed Practical Nurse to join our Medical/Surgical Unit team at Dartmouth Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care, working closely with registered nurses and other healthcare professionals to ensure the best possible outcomes for our patients.

Job Summary

The Licensed Practical Nurse will provide safe, high-quality care to general care patients, working collaboratively with the interdisciplinary team to develop and implement patient care plans. This includes assessing patient needs, providing education and support, and collaborating with the healthcare team to ensure seamless care transitions.

Responsibilities

  • Provide and document nursing care assessments, interventions, and patient education under the supervision of a registered nurse.
  • Review patient plans of care and pertinent active orders in the patient electronic medical record prior to providing care.
  • Assist patients with activities of daily living, nutrition, and hydration according to orders and goals of care.
  • Perform and document focused nursing assessments, including vital signs, pain assessment, skin and wound assessment, functional status assessment, and fluid balance assessment.
  • Administer oral, topical, and inhaled medications, and may administer intravenous medications upon successful completion of NH Board of Nursing approved training and with specific delegation by a registered nurse.
  • Report relevant assessment findings and suggestions to enhance patient care directly to members of the interdisciplinary team in addition to the supervising registered nurse.
  • Comply with policies, procedures, and area-specific expectations related to maintaining a safe environment of care and medication safety.
  • Participate in staff meetings, required learning, attestation, competency, and training activities according to area-specific and organizational expectations.

Requirements

  • NH Board of Nursing Approved IV Therapy Certification required upon hire or within 6 months.
  • Graduate of an accredited school of practical nursing.
  • Licensed Practical Nurse (LPN) in the State of NH required.
