About the Role:

This full-time position requires 1+ years of primary care experience, with an active and unrestricted Pennsylvania license or application in process. You will work autonomously, managing chronic disease, diagnosing illnesses, and implementing plans of care. Collaboration with clinicians, facility leaders, and other stakeholders is essential to deliver effective treatment and ensure seamless coordination.

Responsibilities:

  • Primary care rounding in a skilled nursing facility setting
  • Manage chronic diseases, diagnose illnesses, and implement plans of care
  • Collaborate with clinicians, facility leaders, and others regarding care and services
  • Build relationships with patients, family members, and facility staff to facilitate effective treatment delivery
  • Provide oversight, guidance, and mentorship to nurse practitioners
  • Conduct clinical case reviews, as needed, and share tools, information, and guidelines related to quality of care

Requirements:

  • Active and unrestricted Pennsylvania license
  • Active and unrestricted DEA/prescriptive authority
  • At least 1 year of primary care experience

Compensation and Benefits:

This role offers a productivity-based model (fee for service) that provides control over income potential, with an estimated annual salary range of $180,000 to $200,000. This full-time position is benefit eligible, with access to a comprehensive benefits package including health, dental, and vision insurance, 401(k), disability, FSA, group term life insurance, flex time bank, and employee assistance program.
