Job Summary

We are seeking a skilled Care Manager to join our team. As a Care Manager, you will provide comprehensive care management services to a group of health plan members, creating personalized service plans and coordinating services to meet their unique needs.

Key Responsibilities
  • Care Management Services: Develop and implement effective care plans for assigned members, ensuring high-quality, cost-effective care that meets individual needs.
  • Communication and Collaboration: Build strong relationships with members, families, healthcare providers, and other stakeholders to ensure seamless care coordination.
  • Case Management: Manage a caseload of managed care members, providing ongoing support and guidance to ensure optimal outcomes.
  • Documentation and Reporting: Maintain accurate and timely records of member interactions, care plans, and outcomes, as well as submit required reports to healthcare providers and regulatory agencies.
Requirements

To be successful in this role, you will need:

  • A minimum of one year of nursing experience, preferably in long-term care or managed care settings.
  • A valid New York State Registered Professional Nurse license and current registration.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • A working knowledge of Microsoft Office applications and basic computer skills.
Compensation and Benefits

We offer a competitive salary range of $70,131-$81,275 annually, plus a $15,000 sign-on bonus (restrictions apply), retention bonuses, and a comprehensive benefits package, including medical, dental, vision, 401(k) with employer match, tuition reimbursement, paid time off, holidays, and extended sick leave.
