Job Title: CAP Nurse - Medical Case Manager

Job Summary: We are seeking a skilled CAP Nurse to provide medical case management services to medically fragile children.

Responsibilities:

* Establish eligibility for services and obtain client histories
* Perform initial assessments and develop a plan to provide basic services
* Coordinate services with RN to ensure quality care
* Evaluate and document client programs
* Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team

Requirements:

* Graduation from an accredited School of Nursing
* Ability to direct and carry out prescribed medical treatments and therapeutic programs
* Thorough knowledge of professional nursing theory and practice
* RN certification with one year of nursing experience preferred
