**Job Summary:**

We are seeking a skilled Patient Care Advocate to join our team at Della Infotech, Inc. in providing high-quality patient care services. As a Patient Care Advocate, you will work closely with patients and their families to provide emotional support and assist with navigating the healthcare system.

Key Responsibilities:

  • Provide psychosocial support to patients and their families
  • Assist with resource navigation and coordination of care
  • Screen for suicidality and engage in safety planning as appropriate
  • Complete reports of harm based on state statutes
  • C coordinate with outside entities

Team Dynamics:

The Patient Care Advocate will be part of a dynamic team that works together to ensure seamless patient care. You will collaborate closely with RN Case Managers and other members of the care team to develop and implement effective discharge plans.

Required Skills and Qualifications:

  • Master's degree in Social Work (MSW) or equivalent experience
  • Minimum 2 years of experience in patient care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong problem-solving and critical thinking skills

This role is located in a facility that requires travel. Candidates must be willing to relocate to meet the needs of the position.
