Job Summary

Virginia Mason Franciscan Health Rehabilitation Hospital is seeking a skilled Case Manager to join our interdisciplinary team. As a Case Manager, you will play a vital role in the rehabilitation journey of our patients, providing comprehensive care and support to help them achieve their goals.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the healthcare team to identify and address barriers to care and discharge
  • Communicate effectively with patients and their families to ensure informed decision-making and optimal outcomes
  • Coordinate weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans
  • Review patient records to identify potential missed comorbid conditions and communicate findings to the HIM team
Requirements
  • Masters or Bachelors in Social Work or as required by state
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred
  • Effective oral and written communication skills in English, additional languages preferred
