Job Description:Medical Social Worker
As a Medical Social Worker with Elara Caring, you will have the opportunity to make a real difference in the lives of our patients. You will work closely with our team of healthcare professionals to provide comprehensive care to our patients, addressing their psychosocial, financial, and environmental needs.
Responsibilities:- Assess patients to identify their social work needs and develop a plan to address them.
- Communicate with patients, families, and healthcare providers to ensure continuity of care.
- Implement the plan of care, using patient, family, and community resources.
- Participate in the development and implementation of the Plan of Care to ensure quality and continuity of care.
- Master's Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education.
- 1+ year of social work experience in a healthcare setting.
- Current State License as a Social Worker.
- Excellent verbal and written communication skills.
