About the Role:

Boston Children's Hospital is seeking an experienced APRN to join our Cerebral Palsy Center team. As a key member of our multidisciplinary team, you will provide family-centered care to children with cerebral palsy, working closely with orthopedics, neurology, neurosurgery, complex care, and physiatry teams.

Responsibilities:

  • Participate in interdisciplinary team processes and conferences to identify individualized approaches and planning for patients within the CP Center.
  • Enhance continuity of care by triaging new and second-opinion patients, facilitating and coordinating a comprehensive plan of care in collaboration with patients and families.
  • Function in a direct care role within the Multidisciplinary Clinic, assessing and treating new, returning, and urgent care access for patients in collaboration with the Complex Care Service or other Cerebral Palsy Center Physicians.
  • Support population management to high-risk patients, ensuring recommended surveillance and follow-up.
  • Support perioperative processes with families, including meeting with surgical candidate patients and educating families and managing expectations around perioperative experience.

Requirements:

  • A Master's degree in Nursing
  • A current MA NP license (FNP or PNP)
  • Strongly preferred experiences include: 5+ years of pediatric RN experience and 2-3 years of Advanced Practice experience assisting in the care coordination of patients with complex medical conditions
