About the Role:

We are seeking a dedicated and compassionate Family Care Partner to join our team at UMass Memorial Health. As a Family Care Partner, you will play a vital role in supporting youth and their families in achieving their goals and improving their overall well-being.

Key Responsibilities:

  • Assist in the creation and maintenance of a positive and collaborative working environment where all employees of CHL and enrolled youth and their families feel welcome, supported, and comfortable.
  • Work closely with the clinical hub provider, the family, and the Care Planning Team to monitor the Care Plan and serve as an advocate/liaison for the family when appropriate to access new or different services.
  • Participate in all meetings, training, evaluation/research, supervision, quality management, and related activities as may be directed by the Family Partner Supervisor.
  • Prepare all necessary paperwork such as contact notes, daily service logs, etc.
  • Engage youth and their families in a mentoring relationship, by sharing their own journey, helping to model collaborative interactions between the family and the variety of relevant individuals in the larger system of support.

Requirements:

  • High School Diploma/GED/HiSet.
  • Satisfactory CORI background check.
  • Minimum 1 year's experience working with youth/families.
  • Experience as a caregiver of a youth with special needs.
  • Current, valid, US-issued driver's license and a registered inspected and insured automobile for work-related purposes.
