Salary: The annual salary range for this position is $120,000-$180,000 depending on experience and qualifications.

Job Description:

As an independent optometrist leasing space inside our Sam's Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being. You will have the autonomy to establish your days of practice, hours of operation, control your patient schedule, and set your fee schedule. You will also keep 100% of exam/professional fees.

You can opt into equipment/instrumentation and administrative and/or Opt-Tech support services (if state allows) for a fair market value. To be eligible for a lease agreement, you must have a Doctor of Optometry (OD) from an accredited optometry school and a valid state optometry license or an Ophthalmologist (MD or DO) from an accredited school and a valid state ophthalmology license.

  • A valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • A W-9 form
