Salary

We offer an attractive compensation package with a salary range of $88,000 to $173,200 annually for this role in New York. Pay is based on various factors including but not limited to local labor markets, education, work experience, certifications, etc. Our comprehensive benefits package includes incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).

About the Role

In this rewarding position, you will have the ability to achieve work-life balance with flexible scheduling offered where providers can flex their time over the 7-day work week. No on-call, no weekends, and no holidays are required. As a team member of our HouseCalls product, you will provide annual clinical assessments to patients in the comfort of their homes, helping identify and reduce health risks while coordinating appropriate follow-up care to improve health and well-being.

Key Responsibilities:
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses for care management and active medical management in furtherance of treatment.
Requirements

To be successful in this role, you must possess an active NP or PA license or be able to obtain one by start date, with licensure unencumbered and free of any open/unresolved disciplinary actions. Additionally, you should hold active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification or be able to obtain it by start date.
