Job Description

As a Patient Care Coordinator, you will play a vital role in providing exceptional patient care and ensuring seamless communication between patients, healthcare providers, and our pharmacy team. Your primary responsibilities will include:

  • Managing patient prescriptions and medication therapy plans
  • Collaborating with healthcare providers to ensure accurate and timely communication
  • Providing education and counseling to patients on medication management and adherence
  • Resolving medication-related issues and concerns
  • Supporting our pharmacy team in maintaining accurate and up-to-date patient records
Required Skills and Qualifications

To be successful in this role, you will need:

  • High school diploma or equivalent required; pharmacy technician certification or experience preferred
  • Excellent communication and problem-solving skills
  • Ability to work effectively in a fast-paced environment
  • Strong attention to detail and organizational skills
  • Ability to maintain confidentiality and handle sensitive patient information
