We are seeking a skilled Nurse Practitioner to join our team in Coconino County, AZ. As an In-Home Health Assessment Specialist, you will provide annual clinical assessments to patients in the comfort of their homes.

This is a unique opportunity to make a meaningful impact on the health and well-being of our patients. You will have the ability to achieve work-life balance with flexible scheduling offered where providers can flex their time over the 7-day work week. No on-call, no weekends, and no holidays are required.

Key Responsibilities:

  • Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate findings with the patient's primary care physician to inform potential gaps in care.
  • Identify urgent and emergent situations and intervene appropriately.
  • Educate patients on disease processes, medications, and compliance.
  • Travel up to 85% of the time.

Requirements:

  • A bachelor's degree in nursing or a related field.
  • Active NP or PA license or the ability to obtain by start date.
  • Active national NP or PA certification or the ability to obtain national certification in the state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs.
  • Access to reliable transportation for home visits.
