**Job Summary**

We are looking for a compassionate and skilled LPN to provide quality patient care and perform technical skilled care in the patient's home. As a member of our hospice team, you will work closely with physicians, staff RNs, and other healthcare professionals to ensure the highest level of care for our patients.

**Responsibilities**

* Perform skilled nursing services under the supervision of a staff RN daily.* Administer prescribed medications and treatments as permitted by state regulations.* Assist the physician and/or staff RN in performing specialized procedures as needed.* Assist the patient with daily activities and encourage appropriate self-care daily.* Report to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed.* Perform complex wound care if in-service is documented for specific procedure daily or as needed.

**Requirements**

* Unrestricted licensure in state of practice.* Current CPR certification required.* Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.
