Job Title: Experienced Case Manager for Patient-Centric Care

We are seeking a seasoned Registered Nurse (RN) with extensive experience in case management to enhance patient care services at Mass General Brigham.

About the Role:

This is a full-time, ongoing position with an 8-hour day shift. As a RN Case Manager, you will work collaboratively with the healthcare team to evaluate and predict patient needs, ensuring seamless care coordination and efficient resource allocation.

Key Responsibilities:
  • Evaluate and assess patient needs through continuous monitoring and analysis of medical records.
  • Collaborate with healthcare teams to develop personalized care plans that cater to individual patient needs.
  • Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care delivery.
Requirements:
  • RN Case Manager experience required, preferably in Med/Surg or similar settings.
  • Licensed RN in the state of Massachusetts, with a strong understanding of local healthcare policies and regulations.
  • Minimum of 5 years of experience in acute hospital settings, with a focus on case management and patient care coordination.
  • ADE certification preferred, but not mandatory.
