Join our dynamic team at Sea Mar Community Health Centers as a Healthcare Transition Coordinator. As a Healthcare Transition Coordinator, you will be responsible for coordinating care transitions for patients who have been seen in an emergency room and/or have been discharged from a hospital/long-term care facility. You will also participate in daily huddles to anticipate patient clinical, social, and behavioral health needs.

About the Role
  • Job Summary: Full-time Healthcare Transition Coordinator position available for our Tacoma - Cushman Medical Clinic.
  • Salary Range: $21.88 - $27.00 per hour, depending on experience.

This position requires excellent communication and interpersonal skills, ability to work respectfully and professionally with the community, patients, families, and staff. The ideal candidate will have medical assistant training or equivalent experience in a community health setting.

Responsibilities
  • Work with the care team to identify gaps in care and resolve them using process improvement strategies.
  • Advocate for patient services with community, social service, and medical providers.
Requirements
  • Education: Medical Assistant Training or equivalent experience.
  • Experience: One or more years of experience in a community health setting or family practice.
