About the Role

We are seeking a skilled Veteran Healthcare Advocate Professional to join our team at Volunteers of America Los Angeles.

Job Summary and Purpose

The Senior Health Care Navigator will serve as a liaison between Volunteers of America and the VA or community medical clinic, working with Veterans who require assistance accessing healthcare services or adhering to healthcare plans. This role involves connecting Veterans to VA healthcare benefits or community healthcare services, providing case management and care coordination, health education, interdisciplinary collaboration, and administrative duties.

Duties and Responsibilities
  • Non-Clinical Assessment: Conduct assessments of Veterans in collaboration with the interdisciplinary treatment team, Veteran, family members, and significant others to understand their situation, potential barriers to care, causes, and impact on their ability to access and maintain healthcare services.
  • Health Care Team and Veteran Communication: Work closely with Veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision-making of their care.
  • Serves as a Resource for Education and Support: Provide education and support for Veterans and families, helping identify appropriate and credible resources and support tailored to their needs and desires.
  • Specialized Case Management and Care Coordination: Provide comprehensive case management and care coordination across episodes of care, coordinating case conferences, consulting with service teams, monitoring progress of clients, and assisting clients to meet agreed-upon goals.
  • Acts as an Advocate for Clients: Serve as an advocate for clients before judicial, community, social service, and administrative bodies as needed, facilitating support groups and client participation as appropriate.
Required Skills and Qualifications
  • Minimum 3 years' experience working with veterans in a community/veteran's resource program.
  • Bachelor's Degree (Social Work, Public Health Administration, or related) OR four years' direct relevant experience.
  • Master's Degree in related area preferred.
  • LCSW Preferred.
  • U.S. Veteran strongly preferred.
  • Existing relationships with VA or other community/veteran resource providers.
  • HMIS Database experience strongly preferred.
Salary and Benefits

The estimated salary for this position is $23.83 - $25.08 per hour, based on location and experience. We offer competitive medical, dental, vision, and retirement benefits.
