This is a unique opportunity to work with patients in their homes, providing personalized care and support. As a Healthcare Specialist, you will be part of a multidisciplinary team that provides rehabilitative care and skilled nursing services to clients.

About the Role
  • Evaluate patients' functional status and develop individualized intervention plans to achieve favorable outcomes.
  • Assess cognitive, psychological, and motor aspects of performance and identify equipment needs and environmental adaptations.
  • Educate patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
Requirements
  • A current Vermont Occupational Therapist license.
  • Prior home care experience preferred but not required.
  • A minimum of one year of recent work experience as an Occupational Therapist.
