Service Coordinator Specialist

**Job Summary**
As a Service Coordinator Specialist with the Stark County Board of Developmental Disabilities, you will play a critical role in ensuring that individuals with developmental disabilities receive the support and services they need to live their best lives. **About the Role**
The primary focus of this position is to coordinate and manage the services and supports for individuals with developmental disabilities. This involves assessing individual needs, developing personalized service plans, and working with families, guardians, and service providers to ensure seamless delivery of services.
This is a full-time, benefits-eligible position that requires a strong commitment to providing exceptional customer service and supporting individuals with developmental disabilities.
**Responsibilities**
- 75%: Core Services
- Conduct comprehensive assessments of individual needs and develop personalized service plans that align with their goals and aspirations;
- Coordinate and manage services and supports from various providers, including waiver service providers and community resources;
- Provide ongoing monitoring and evaluation of services to ensure they are meeting individual needs and achieving desired outcomes;
- Work collaboratively with families, guardians, and service providers to address any challenges or concerns that may arise;
- Document all services and interactions with individuals and families in accordance with agency policies and procedures;
- Develop and maintain effective relationships with service providers and other stakeholders to ensure seamless delivery of services.
- 20%: Secondary Responsibilities
- Maintain accurate and up-to-date records of individual service plans, progress notes, and other relevant documentation;
- Ensure compliance with state and federal regulations, as well as agency policies and procedures;
- Participate in on-call rotations to provide crisis assistance and support to individuals and families as needed;
- Collaborate with other team members to ensure consistency and quality in service delivery.
- 5%: Miscellaneous Responsibilities
- Assist individuals with due process issues, such as requests for appeals or grievances;
- Participate in training and professional development opportunities to enhance skills and knowledge in areas related to service coordination and disability support;
- Represent the agency at community events and meetings, as needed.
**Requirements**
To be successful in this role, you will require:
- A bachelor's degree in a related field, such as social work, psychology, or special education;
- Experience working with individuals with developmental disabilities, either in a direct care or administrative capacity;
- Strong communication and interpersonal skills, with the ability to build effective relationships with individuals, families, and service providers;
- A valid Ohio driver's license and acceptable annual BMV check;
- Proof of insurance and a good driving record.
**Benefits**
We offer a competitive salary range of $45,000 - $60,000 per year, based on experience, plus a comprehensive benefits package that includes medical insurance, dental insurance, life insurance, flexible spending, and vision benefits.
